What choices do I have for Effective Dates?

On the enrollment application, you can select either the 1st or the 15th of the month for the effective date for membership and any optional supplemental membership plans.

Note: Since we do one bill for all membership plan(s) you enroll, you must choose a matching effective date (1st or 15th) with your original membership effective date if you are adding additional supplemental membership plans.

Example: If you already have a UBA Membership or HAA Membership with a 1st effective date, your additional supplemental Membership plans or a la carte Benefit Boost Subscription plans must also be a first effective date.  Any questions, contact us at 866.438.4274.

 

*Note: Since Benefit Boost Subscription Plans can be sold stand-alone and do not require membership, you can select either a 1st or 15th effective date. The rule above only applies if you are adding a membership with UBA or HAA and have additional supplemental plans. All plans in that circumstance in order to do the single billing would require the same effective date.
