Unfortunately, you WILL NOT be eligible to use the Dental Insurance for Major Services within the first 12 months from the effective date for all members. All Major Services are 50% Coinsurance In- or Out-of-Network. Diagnostic & Preventive Services and Basic Dental Services do not have the 12 month waiting period and are available to use after your effective date. Major Services include:
Surgical Periodontic Procedures (to treat gum disease)
Endodontic Services (i.e. Root Canals)
Complex Extractions & Dental Surgery (all other Oral Surgery services)
Treatment of Temporomandibular Joint Dysfunction (treatment is limited)
Crowns & Veneers (Major Restorative Services - Crowns & Veneers)
Once per tooth in a 5 year period
Bridges, Implants & Dentures (Prosthodontic Services - bridges & dentures)
Once per tooth in a 5 year period (Implants - once per tooth per lifetime)
Relines & Repairs of Bridges & Dentures
