How do I update my Credit Card Information that I use to pay my Membership Plan(s)?

If you need to update your Credit Card number, the Expiration Date or CVV code to be able to process your payment for your UBA Membership, Benefit Boost Subscription or HAA Membership plan(s), you can do it using the following methods:

• Call us at 866.438.4274; and/or
• Use the Credit Card Change Form below to update and then send back to us Securely.
