If you have an optional supplemental UBA Gap Product that includes Group Accident Insurance underwritten by United States Fire Insurance Company and you have a Claim, submit a claims to the claim to the claims administrator. The claims administrator (CBP) will get all the necessary documentation and make sure that all other insurance paid already has been accounted for. When they determine the amount payable for your claim, you will be paid one of the two ways below:
- If you paid the provider directly, be certain to include proof of payment to the claims administrator at the time you submit the claim. Otherwise, the claims administrator may pay the provider instead of you.
- If the providers in question included an an assignment of benefits on their itemized bills, the claim will be paid directly to the provider.
Eligibility will be determined by the plans, limitations, terms & exclusions.
Don't forget that the Group Accident Medical Expense Insurance is secondary, meaning that all other insurance pays first. You are eligible for any additional covered expenses that were not covered by your other insurance up to the benefit maximum minus the $100 deductible per accident.
