The member portal will show you exactly the claim form you need to download for the type of insurance you have purchased and the type of claim that you experienced. By using the member portal, you ensure that you are downloading and submitting the correct form to the correct claim's administrator.

You look at the Type of Group Insurance and the across from it on the right side is the Claim's Administrator Phone Number where you can check on claims, check on eligibility, etc for the product that you have purchased. You will also locate the Product Name, Product ID and Plan Effective Date above that section. Depending on the product and the state, the insurance carrier partners that underwrite the group insurance programs with UBA could vary.
