Where do I find the right Claim form for the group insurance in My Membership Products?

The best way to make sure you are getting the correct claim form for the group insurance in Your specific membership product and for your state, download the claim form from the Member Portal.

The member portal will show you exactly the claim form you need to download for the type of insurance you have purchased and the type of claim that you experienced. By using the member portal, you ensure that you are downloading and submitting the correct form to the correct claim's administrator.

 

Do you have questions about what you need to send in, you can either contact the Claim's Administrator for the insurance you are filing the claim form (the number for the claim's administrator is on the back of the ID card) or you can contact us at 866.438.4274 and we can help you find out what you need.

 

Important: Healthy America, United Business Association (UBA), and Healthy America Association (HAA) are not authorized to pay claims or be involved in the claims decision process.  All claim decisions are made by the TPA claim's administrator and the insurance carrier. (Note: Healthy America Association does not have any optional supplemental Membership Products that include group insurance at this time.)

About the back of the ID card above: 

You look at the Type of Group Insurance and the across from it on the right side is the Claim's Administrator Phone Number where you can check on claims, check on eligibility, etc for the membership product that you have purchased. Below under Underwriter of Products you will see which underwriter is the underwriter for the Type of Insurance in the section above. The Type of Insurance will have one of the 4 symbols that will match with the symbols of the underwriter for that type of insurance: *, ^, #, or ~.

At the top of the card, You can locate the Plan Name, Plan ID and Plan Effective Date above that section. 

 

HAA MEMBERSHIP PRODUCTS

Note: Currently, HAA does not offer any optional supplemental group insurance products at this time. When group insurance membership products become available with HAA, we will update this section to include the back of the HAA Membership ID card as an example.
